We are a group of caring individuals who just want to make a difference in the lives of those at risk or suffering from the crisis of homelessness.

WE SEEK TO AID ONE FAMILY AT A TIME BY PROVIDING THEM WITH COMPREHENSIVE TRANSITIONAL RESOURCES THAT LEAD THEM TO SUSTAINABILITY.
SERVICES - HOW WE DO IT
Affordable Living Open Hearts Homes gender specific facilities cater to youth, elderly, and families at risk of homelessness. We are LGBQT and HIV friendly and provide diverse resources to assist with enhancing the lives of all participants. life skills development Our programs support adaptive and positive behaviors enabling participants to deal effectively with the demands and challenges of life. Open Hearts Homes focuses on the entire well-being of an individuals seeking to reintegrate them as active and productive members of their communities. case management and sustainability resources Open Hearts Homes implements a collaborative process of assessment, planning, facilitation, care coordination, evaluation and advocacy to provide options and implement services that meet individual and families' comprehensive needs and that promote participant vitality, quality of care, and productive outcomes.
